Do You Actually Feel Good? Conventional vs. Functional Lab Ranges in Your 40s

Every day I talk to women in their 40s who have ‘normal’ lab results but still wake up every morning knowing that something’s off in their body. Traditional medicine often tells them to ignore it. That if their labs are normal, they’re fine. Nothing to worry about here. 

That’s just not true. 

Traditional lab reference ranges are established by testing large populations of people and identifying where the middle 95% of results fall — meaning the bottom 2.5% and top 2.5% are flagged as "abnormal." On the surface, this sounds reasonable, but there's a critical flaw: those populations often include people who are already unwell, sedentary, or nutrient-deficient, so "normal" simply reflects what's common, not what's optimal. For women in perimenopause, this is especially problematic, because symptoms like fatigue, brain fog, weight gain, and mood changes can be very real and disruptive even when labs fall comfortably within standard ranges.

Functional medicine ranges, by contrast, are designed around what research shows to be optimal for health, energy, and longevity — not just the absence of diagnosable disease. They tend to be narrower, and they're used as a tool for early intervention rather than waiting until a value crosses into territory that warrants a prescription. For a perimenopausal woman, this distinction can be the difference between being told "everything looks fine" and actually getting answers. 

There are some standard labs that your doctor will most likely order during a routine physical. There are additional labs more commonly ordered in the functional space that really help perimenopausal women understand the full picture of their health.

Review your own lab results against these ranges and maybe even come prepared to your next yearly physical with a few questions or suggestions of additional labs to run.
